Insurance & Intercollegiate Sports
The student insurance plan has a provision for intercollegiate sports injuries.
How Student Health Insurance Works When You Have an Intercollegiate Sports Injury
All student athletes who are participating in intercollegiate sports, sponsored by the Policyholder, are covered for any sports Injury with United Healthcare as follows: Benefits will be paid under the Schedule of Benefits for intercollegiate sports Injury up to $4,000 for each Injury.
Students can go to any provider, however selecting providers or facilities that participate in United Healthcare Options PPO network ensures the highest level of coinsurance.
- Preferred Provider Coinsurance: 100%
- Out of Network Coinsurance: 80%
Click HERE to locate in network physicians, hospitals or other facilities. There is a $30 co-pay/deductible for physician visits regardless of whether you use a preferred provider or go out of network.
What Should I do if I'm Injured While Playing Intercollegiate Sports?
- Seek immediate medical treatment.
- Present your United Healthcare insurance ID card to the provider.
- Complete an injury detail form (available in Athletics). United Healthcare cannot process your claim until they receive this information.
What if I Have Other Insurance Coverage?
The student insurance plan with UHC is primary, therefore claims should be submitted to the address on your insurance ID card.
