2012 - 2013 Insurance Plan FAQs
The 2012-13 Student Health Insurance Plan is effective August 1, 2012 and runs through July 31, 2013. If the student attends Oxy both semesters the coverage would end on July 31, 2013. This is a Preferred Provider Option (or PPO). When care is received from a Preferred Provider (in-network), acovered medical expenses will be paid at the Preferred Provider level of benefits. In all other situations, reduced or lower benefits will be provided when an out-of-network provider is used.
All covered services provided by Emmons Health Center are paid at 100% and not subject to copays and deductibles. There is no charge for physician/nurse practitioner visits when students are seen at the Health Center however, the Health Center will charge for vaccinations, medications, exams and procedures. In addition, prescriptions filled at the Health Center will be paid at the in-network levels of co-insurance with no co-pay. Emmons Health Center will automatically bill United Healthcare Insurance Company for any charges incurred at the Health Center (any un-reimbursed balances will be charged to the student's Oxy account) on behalf of the student.

Insurance Premium
$391.25/semester = $782.50 Annual (this is for twelve consecutive months of coverage). The student health insurance is mandatory and cannot be waived.
Effective Date
The 2012-13 Student Insurance Plan becomes effective on August 1, 2012. This plan covers students any time, anywhere, during semester breaks, summer vacation and while studying abroad.
Insurance ID Cards
United Healthcare health insurance ID cards will be mailed to the students' campus mailbox. A copy of the student's ID card is also available online. To print a copy of your insurance ID card:
- Go to Gallagher Koster
- Your User Name is your Occidental College email address.
- Your Password is your Student ID number.
- Once logged in select "Account Home," then click on "Generate ID Card."
Students should carry their United Healthcare insurance ID card with them at all times and present it to providers any time the student seeks healthcare. This is your primary insurance coverage so claims should be submitted to United Healthcare first.
Finding an In-Network Provider
Students can go to any provider, however, they can save money by seeing providers that participate in the United Healthcare Options PPO network. To locate a network provider or pharmacy click HERE.
Claims
Write your Student Insurance ID number on all claims before submitting them to United Healthcare Student Resources. Any fees incurred at Emmons Health Center will be submitted to United Healthcare Insurance Company on behalf of the student. Claims can be submitted to:
