Vehicle Request and Authorization Form

The Vehicle Request and Authorization Form can be obtained from the Facilities Management office during normal business hours. Drop by to pick up the form or email vehiclerental@oxy.edu to have the form sent to you. 

 

Passenger Manifest

The Passenger Manifest will be provided when you pick up your vehicle.
