Checklist & Timeline

Due as soon as possible: 

• Request final official transcripts from your most recent college
• Activate your Oxynet computer account using your activation code
  • Don't know your activation code? Call the Office of Student Life at 323-259-2918 (NOTE: Activation codes will be issued to the student only, and will not be issued via email, so you must call!)
• Activate your oxy e-mail through Oxyconnect (Google Apps for Oxy) and check it often, as your oxy email is the official correspondence you will receive from the College
• Activate authorized users (parents or guardians) on your online student business services account to receive your electronic billing statements (Click on Student Services tab)
• Check for updated information on student health insurance requirements
• Meal Plan Selection (Click on Student Services tab) 

Due by January 7:

• Master Promissory Note for Perkins and Oxy Loans (if applicable)
• Loan Entrance Interview (if applicable)
• Request for Accommodations Form (if applicable)

Due by January 14:

• Advising Information Form
• Online language placement exams for French, Spanish, and German (if applicable)
• Complete Math Placement Self - Assessment Form (if applicable)
• Chemistry Placement Exam (if applicable)
• Alcohol-Wise (completed by student under the age of 21)
  • Instructions can be found on page 2 of the New Student Packet (NOTE: the dates in the New Student Packet reflect Fall Orientation deadlines. Alcohol-Wise will be available from January 7-14 to complete. You must complete Part I of Alcohol-Wise by January 14, or you will not be able to register for classes!)

Due by January 18 (you can bring forms with you to Orientation): 

• Housing and Meal Plan License Agreement (you will receive this form upon move-in)
• Religious Information Form (optional)
• Waiver FERPA Privacy Rights Form
• Student Photo Release
• Health Forms:
  • Health History (completed by student)
  • Financial Responsibility and Permission for Treatment (signed by student and parent/legal guardian)
  • Physical Exam (completed by health care professional)
  • Immunization Record (completed by health care professional)
  • Authorization to Share Health Information
  • Mutual Arbitration Agreement
• Complete Clearance Form (click on Student Services tab)
