Emmons Health Center Forms Policy

Students are responsible for knowing the following:

All entering students are required to have completed the following forms and sent them to Emmons Health Center prior to registration for class: 1) Health History, 2) Financial Responsibility and Permission for Treatment, 3) Physical Examination (either Sports or Entrance Physical is acceptable, 4) Immunization Record, 5) Authorization to Share Health Information. and 6) Mutual Arbitration Agreement. Completion of all health forms prior to matriculation are strictly enforced as to ensure the health and well-being of all Oxy students. An Emmons Health Center staff person will contact students who neglect to fulfill this requirement. Failure to comply with this policy may result in the College fine of $100.00 and other measures.
