Application Checklist

The following are required forms to apply to Occidental.  Where noted, certain forms are specific to either the Occidental Application or the Common Application.

	Form A1 (Occidental Application Only)
	Form B1/Common Application
	Application Fee/Fee Waiver ($50 payable to Occidental College)
	Form C/School Report Form
	Form E1, E2/ Teacher Recommendations (Note: recommendations submitted from 9th grade teachers will be considered as supplementary information)
	From D/Mid-Year School Report
	SAT or ACT (with writing) scores
	Common Application Supplement
