Student Health Insurance Plan

All Occidental College students are automatically enrolled the College-sponsored Student Health Insurance Preferred Provider Option Plan through United Healthcare Student Resources.

The Student Health Insurance Plan coverage begins on August 1, 2011 and runs through July 31, 2012. If care is received from a Preferred Provider (in-network), any covered medical expenses will be paid at the Preferred Provider level of benefits. In all other situations, reduced or lower benefits will be provided when an out-of-network provider is used.

Services provided by Emmons Health Center are paid at the in-network level of co-insurance. There is no charge for physician/nurse practitioner visits when students are seen at the Health Center, nor are they subject to a co-pay, however, the Health Center will charge for vaccinations, medications, exams and procedures. In addition, prescriptions filled at the Health Center will be paid at the in-network levels of co-insurance with no co-pay. The Health Center will automatically bill United Healthcare Insurance Company for any charges incurred at the Health Center (any un-reimbursed balances will be charged to the student's Oxy account.) on behalf of the student

INSURANCE PREMIUM
The Insurance Premium for the 2011 - 2012 Year is $713.30 ($356.65 per semester). Student Accounts automatically bills the student insurance premium to the student's account.

EFFECTIVE AND TERMINATION DATES
The Student Insurance Plan becomes effective on August 1, 2011 and the coverage ends on July 31, 2012. This plan covers students worldwide, when they are on breaks or traveling.

INSURANCE ID CARDS
ID cards will be mailed to the students' campus mailbox.  ID cards are also available online within 24-48 hours upon Gallagher Koster's receipt of your enrollment in the plan and can be printed per your request. To print a copy of your insurance ID card:

1. Go to www.gallagherkoster.com/oxy.
2. Log in using your existing account information (first time visitors will need to create an account).
3. Select "Authorize Account" located to the left of your screen under Account Information and enter your Student ID number along with your date of birth.
4. Once your account has been authorized, select ‘Account Home' and click on ‘Generate ID Card'.

Students should carry their United Healthcare insurance ID card with them and present it to providers in the event of having to access healthcare services. If the student has other insurance coverage, the coverage with United Healthcare is primary.

FINDING A PROVIDER
You can go to any provider, however, you can save money by seeing providers that participate in United Healthcare's PPO network. To locate a network provider or pharmacy, go to www.uhcsr.com. Go to Find My School's Plan. Enter Occidental College. click on "Find UHC Options PPO" or "Pharmacy Locator". You can also go to www.gallagaherkoster.com/oxy

CLAIMS
Any fees incurred at Emmons Health Center will be submitted to United Healthcare Insurance Company on behalf of the student.
