If you have a good rapport with a faculty member or your academic advisor you can request that they write a letter of recommendation

Who to contact to fill out your Midterm Report

Some institutions require you to submit a Midterm Report. If you are applying to an institution that does require that each of your instructors for your courses this semester will need to sign your Midterm Report. You should fill out the top part of the form and then ask your instructor either to send it back to you or send it to the institution you are applying to. Many instructors will prefer to send it directly to the institution, and if so it would be easiest to send each of your instructors their own form.
